FLUZONE is given to all children 6mo’s – 17 years. This is also a quadrivalent vaccine
Only on the dates listed or, at the time of a regularly scheduled in-office visit
NO fever, cough, sore throat or shortness of breath or exposure to any person with COVID symptoms within 14 days
Masks will be required at all times
